1. How long have you been losing captions on ___-TV ##-1 and ##-2? Is this new, or has it been happening for a while?
2. How far is your home from the ___-TV broadcast tower in aerial miles? This distance is easy to obtain from AntennaWeb.org.
3. Is your antenna indoors or outdoors?
4. How many feet above ground level is your antenna installed?
5. Are there a lot of leafy trees or a tall building really close to your house?
6. What brand of antenna do you have and approximately how many inches or feet long is the antenna?
7. Do you know the model # of the antenna? If it’s hard to find the model #, could you email us a photo of your antenna, please?
8. Does your TV signal feed directly out from the antenna and into your DTV set over a coaxial cable, or is there any equipment installed in between your antenna and your DTV?
9. What brand of DTV do you have?
10. What year was your DTV manufactured?
11. On your DTV, do you have the captions source of ‘CC1’ or ‘Source 1’ selected?
12. Do you watch ___-TV during the day or early evening? If so, have you noticed a caption outage during the day or early evening?
13. Are there any of our shows that you watch that are always missing the captions? (If so, we can check if how the captions are formatted for that particular show may be an issue.)
